Need help Identifying computer part

It is currently broken and I want to buy a replacement. But, I cannot find it anywhere online! I dont even know what to search for! Please Help!

Those look like front panel ports. What makes you think they're bad?

They are the front audio and USB connecters, you can use almost any replacement as long it will fit. Is it from a lappy?

Im only a year late, well not quite a year but I was wondering if anybody ever figured out how to get another one of these. I too have a bad one. It is a audio/usb/firewire board that came on an Coolermaster nvdia case. The one of the usb ports got busted and it kind of caused the whole card to go flaky so I was looking for a replacement and I can't find anything other than some chinese inventory catalogs and this website. Any offerings would be helpful.
