Default Making an Icon?

Does anybody know how to make an Icon for Windows Vista? Ya know, one that actually scales up when you make the icons bigger? I've tried making icons in gimp, but they come out all pixely and ugly somehow and they stay stuck at 32x32 or whatever and don't scale up with other icons. I've tried following lots of tutorials, but no matter what I do, my icons just don't come out like their's do. What am I doing wrong?

I'm making them 256x256. That's what Vista icons go up to right?

Dunno if it support Vista but i used Icofx for all the icons i've ever done. Nice little bit of freeware.
