mail in rebates

its my first time doing any mail in rebates from newegg. i need help.

i bought a zotac 8600gt and this is the mail in rebate form i have to do.

it asks for an invoice instead of an order confimation? were do i g et this invoice?

it also wants me to put the name of the person on the invoice onto the mail in rebate form. but my shipping address' person and billing address' is different...

whose name shud i put down?

The invoice should be with the package once the items are received. If you have a Newegg account, you can log in and print your invoices. As far as the name, I'd put the billing name.
 
Don't forget to put the UPC with the rebate. I don't think it's required, it just gets thing done faster.
 
I had 3 or 4 mail-in rebates with my laptop (and the stuff bundled with it) and I never did get my $75 rebate from Lexmark for the bundled printer that I didn't use. >_>
 
=/. the rebate tells me to cut out the barcode sticker but that ruins the box. if the merchandise breaks and i need to RMA, wat shud i do?

the box would be cut since i need sticker for rebate...
 
That's why I don't rely on mail in rebates to attain discounts. They make you jump through hoops on purpose, hoping you'll forget or give up.
