Default Unmountable Boot Volume

My kids computer had a blue screen with a error message that said "Unmountable boot volume". I have experienced this before on another computer so I had no problem fixing this. My question is what causes this problem in the first place?

I have seen that already. I know how to fix the problem, I want to know how the files get messed up to begin with.
