Default Fixing Keyboard

We need to change the keyboard inputs on a keyboard. How should we go about removing and replacing them?

Ps/2 to usb adapter.

I assume this is what you want to do:
PS/2 output on keyboard, usb input on mobo. If not, lolwut?
