Hard Drive is Failing?

Ok,The hard drive i'm using is a Maxtor Diamondback 8 40gb IDE hard drive.

While i was on my computer i heard a ticking sound from the hd and my computer just froze up. Restarted my computer and it didnt recognize my hard drive AND my cd drive. Unplugged the hard drive from the computer and plugged it back in and started it. Recognized both drives. But now my computer spends a few minutes longer on the Xp boot skin and when i'm using the internet or just being on my desktop my computer "stutters" its alot slower, also it wont load any of my games, just freezes.

I've tried Disk Error Check but it stops when it gets to Check Disk 4 of 5. Not sure what to do. Should i reformat or what?
 
i installed the program, ran it and i got an unhandled exception error

i think i'm just gonna reformat and go to vista 64 bit this pro 64bit has been unstable since the beginning.

now if i reformat will that fix my hard drive problems or what? i've got my 7600gt comin in and i dont want shit failing on me
