We are required to get our cars inspected once a year here in Pennsylvania. They check things like lights, horn, make sure the tires are good, the exhaust is good, they check the brakes to make sure they are good, check the suspension to make sure nothing is loose and the shocks aren't leaking and whatnot. There also can't be any rust holes in the body. In our area we also have emissions, but it's just a visual inspection, basically they just make sure that all your emissions equipment like the catalytic converter and egr valve are installed. But most bigger city areas have regular emissions where they test the exhaust that is coming out of the car.
