Yep, 13.21V on the 12V rail is too high but just to be sure those are the right readings, boot into the BIOS and check your voltages there. Restart your computer and repeatedly tap the DELETE key. This should bring you to your BIOS screen. And your voltages should be there.
EDIT: To get out of the BIOS, just press ESC.

In the BIOS its running at 11.77 on the 12v rail.

That's better, and it means that the software wasn't reading the temps right. Do you have another PSU you could use?

I haven't, no. Was that just to check to see if it would improve? or is that the problem and I need a replacement?

It would be to make sure that the PSU is the problem, so that you don't go and buy a new one for no reason. Have you got a friend or something who could lend you one?
