hi,
not a genius wen it comes to graphics cards but i think that card has an s video port on the back which wud make the easiest way a simple s video lead assuming ur t.v has an s video port if not a simple scart adaptor shud do it and then u want a 3.5 mm head phone jack - 2 read and white phono

right well i dont know where ur from uk or us
im from uk so i can tell u where to get that from easily depending on what size you need .
here goes

but i have found going through a scart adaptor lowers the quality of text and even makes it hard to read so best if ur tv has a s video socket like the one on the back of your graphics card

again hopefull your t.v has an s video socket on it so u dnt need the adaptor as u will get better quality pictures and text
the audio lead the small 'headphone' looking end goes into the back of your pc where ur regular speaker would go and the red and white would go into the adaptor or your tv.
and the s video cable goes into your graphics card and then either into slot on your tv or into adaptor.
very simple i always have my pc hooked upto my t.v as an extended desktop
